How to tell this guy....?

There's this guy that I've liked since the 4th grade and now its 6 years later and I still havent told him. How do I? I'm pretty sure he doens even remember my name. I'm going to see him tomorrow so any tips would help. I've talked to him a few times and we always hit it off but nothing beyond that. How do I get to know him since our friends never hang out together anyway? We're in two totally different social circles --the downfall of high school :/ Thanks guys :)
